Protein first: how to eat in your first month

When your appetite shrinks, every bite matters more. Three simple rules our providers give every new patient — no macro math required.

The first month on a GLP-1 surprises people — not because eating gets harder, but because it gets so much easier. Meals you used to finish without thinking now fill you up halfway through. That's the medicine working. It also creates the one nutritional risk of these programs: when you're eating less, what you skip matters as much as what you eat.

You don't need a food-tracking app or macro spreadsheet. You need three rules.

Rule one: protein goes first — literally

Your appetite may close the kitchen before you've finished your plate. So order your plate accordingly: eat the protein before anything else. Eggs before the toast, the chicken before the rice, the Greek yogurt before the fruit. When fullness arrives early, you want it arriving after the most important nutrients are already in, not before. Protein is what protects your muscle while the weight comes off — and keeping muscle is what keeps your metabolism working with you instead of against you.

When fullness shows up early, make sure protein already got a seat at the table.

Rule two: drink like it's your job

Much of what we drink for hydration, we drink around meals — and you're having smaller meals now. Nausea, headaches, and fatigue in the first month are more often dehydration than medication. Keep water within reach all day and sip before you're thirsty. If plain water bores you, that's what sparkling water and a squeeze of citrus are for.

Rule three: small plate, real food

A shrunken appetite will happily be filled by whatever's nearby — and if that's crackers, it will be crackers. Build small, real meals on purpose: a palm of protein, a fist of vegetables or fruit, and enough fiber to keep everything moving (this matters more than anyone warns you about). Skipping meals entirely feels efficient but tends to backfire as fatigue and, later, muscle loss.
